Also, if you are interested in making any sport an official PSAL sport on campus, you can apply with Dr. Ally after conducting a successful club for 1 year.
PSAL Medical Form and Parental Consent Form
All student-athletes will be required to fill out the PSAL Medical Form and PSAL Interscholastic Athletics Parental Consent Form to participate in any team sports-related activities.
In order to attend ANY pre-tryout or tryout, the paperwork must be in before the tryout date.
Bring all paperwork on the first date of pre-tryouts or tryouts that you attend. You will not be permitted to attend/participate if you do not bring the hard copies.
You must be cleared before you attend/participate in any team sports-related activity.

Each student-athlete is required to have a physical and parent consent form on file. Each athlete is also screened for 90% attendance and meet academic eligibility requirements.
